Please read the following agreement before submitting your form

Should you be considered for appointment, you will be required to submit ID for TNH to apply to the Australian Federal Police (AFP) in order to perform a Police Check. The cost of the check is to be paid by the TNH.

As a volunteer of Tully Nursing Home I agree that I shall not, during the period of my volunteering disclose or use in any manner whatsoever any resident or organisational details of employment with the service.

I am bound by the Privacy Act 1988 and the National Privacy Principles 2001. I understand I am not obliged to disclose personal information about an individual unless that individual has consented to that information.

Upon termination of my volunteering, I shall immediately deliver any property to Tully Nursing Home which is in my possession or under my control which in any way relate to the business of Tully Nursing Home or its residents past or present.

I acknowledge that if this agreement is breached, Tully Nursing Home may take appropriate disciplinary action including termination of volunteering

I further understand that this statement will be part of my volunteer file
